For a mailed state request, use ADPH’s mail-in application and send it with a check or money order payable to Center for Health Statistics to P.O. Box 5625, Montgomery, AL 36103-5625; do not send cash. In Alabama, any county health department can issue an available certified divorce certificate, often while you wait. For online or telephone ordering, follow the ADPH-linked VitalChek route; vendor service and upgraded UPS shipping carry additional charges.
